The continuum of care by CrownPointe Communities standards is designed to meet the various levels of services deemed necessary by seniors, their families and physicians. By developing campuses with a variety of care and housing options, a senior can utilize various services both on the campus and within the community. Transportation is one of the services available to residents to ensure that they can safely leave the campus to obtain community-based services and to participate in planned activities.

Brief illnesses or surgeries may require convalescence in a setting that offers more than assisted living or independent living can provide. In those cases, CrownPointe Communities provides options to residents and families that include various community-based medical options. CrownPointe Communities has developed strong working relationships with medical facilities and various support service groups in all of its communities, and these can be coordinated through our management teams at every location. These could include diagnostics through local hospitals, home health, durable medical equipment, private care services, as well as various out-patient services. This network of services provides each resident with a seamless continuum for all services generally required by active seniors.

A warm and healthy environment, combined with personal enrichment programs for all levels of skill and interest, help ensure that our residents meet their highest level of physical and emotional well-being. Each campus is committed to providing a “home-like” atmosphere for each resident in terms of housing, meals and social activities.
